Mission: To promote and improve the quality of life and to eliminate factors leading to poverty for northern new jersey residents by providing comprehensive health and social services with compassion, through collaborations and partnerships in a culturally sensitive environment.
Programs: Healthcare services: health care services are provided from 11 sites strategically located throughout hudson, bergen and passaic counties. These services include primary and preventive health care for both the uninsured and under-insured predominantly minority residents, and covers prenatal, gynecological, family planning, pediatrics, and adult medicine as well as dental treatment, mental health, and addiction services counseling. In addition, mobile health and dental units provide a variety of vital medical screenings, wic (women, infants & children) enrollment, and dental care. The agency continues to focus on reducing unnecessary hospitalizations and use of emergency rooms. In 2016, 71,666 unduplicated patients were assisted through 268,430 professional visits. Eighty-one percent (81%) of patients are latino and 68% are sufficiently limited in their english that they were served in another language. Ninety-three percent (93%) had incomes at or below 200% of the federal poverty level (fpl); and of that number, 78% fall at or below 100% fpl. Uninsured patients made up 43% of the patients, and 44% were covered by medicaid; this includes 10,428 chip. early childhood development and child care: funded to serve 397 pre-school children, 96 infants and toddlers, and 28 pregnant women, the north hudson community action (nhcac) head start/early head start (hs/ehs) programs provide full-day, full-year early education and parenting programs, integrated with extended childcare, health, and family support services. 92 % of families served this fiscal year had incomes at or below 100% of poverty and 202 children participated in pre-school activities and are developmentally ready to enter kindergarten.
community services: encompasses all programs not included in the other 2 categories. A. Women, infants & children (wic) nutritional program - wic nutritional education and assistance services continues to provide supplemental nutritious foods to pregnant, breastfeeding and postpartum women, infants and children up to the age of five. The program also includes nutrition education and counseling, breastfeeding promotion and support, immunization screening and health care and social service referrals, as well as vouchers. These vouchers are redeemable for specifically designated foods at participating supermarkets. Total clients served: 11,611employment services/job placement - assessment and referral to employment plus job development accompanied by job readiness services for those needing such aid before beginning job search. Total participants enrolled: 1,155total jobs received: 466hispanic affairs/immigration & naturalization - a variety of assistance related to entrance, legal residency, citizenship and other ins concerns including special projects, e. G. , family unification visas and work permits; amnesty and lottery programs. Total clients served: 1,172homes for heroes - an 18-unit supportive housing program directed to the needs of disabled homeless veterans in hudson county. Total housing units occupied: 18 supportive service for veteran families (ssvf) - provides client-centered case management and other services (health and dental care, mental health, substance abuse counseling, employment, housing counseling, financial education, etc. ) to ensure low-income veteran families receive the ongoing attention and care they need. Short-term financial assistance (legal, child care, transportation, security payments, rental assistance, utility payments and moving costs) may be available to eligible veteran families to assist with staying in or acquiring permanent housing on a sustainable basis. Total veterans served: 122housing counseling/tenant advocacy - provides education and assists residents with pre-purchase/home buying, rental counseling, shelter/services for the homeless, and tenant advocacy. North hudson community action corporation is a hud approved agency with a certified housing counselor. Total clients served: 96hud coc rapid re-housing - provides quick entry into housing paired with temporary financial assistance and individualized case management that assists a minimum of 22 households with their housing stability goals. A number of support services (job placement, parenting skills workshops, financial management, etc. ) are provided to enable self-sufficiency after the temporary assistance ends. Total clients served: 44project shape - (social & health assistance program for the elderly): provides a full spectrum of social and home health services for area residents 60 years old and over. These services include information and referral, benefits screening, access assistance to entitlements and other forms of care, case management, homemaker assistance, and management of the jacc program. Total clients served: 1,461safe housing/home repair (residential maintenance) - provides a maximum of $500 in home repairs to correct health and safety violations for seniors 60 and over. Total clients served: 6christmas toys - annual toy distribution to children in neediest families served; additional toys were purchased to support those impacted by hurricane sandy. homeless prevention & housing services include:homeless prevention: temporary housing for eligible tanf recipients, the working poor, and non-welfare families for 6 to 12 months. Case management and support services, in cooperation with hudson county welfare for tanf families are also provided. 6 tanf recipients and other qualified families can be served at any one time; the capacity for working poor families is 11.
