Programs: National community health partners (nchp) housing for heroes (hfh) federal project is dedicated to housing homeless veterans. The hfh project is unique in that it follows the housing first model enabling veterans to secure housing and receive supportive services as needed via case management. A plethora of issues surrounding hfh clients ptsd, substance abuse and mental health, legal, tbi and hiv-positive. From july 1, 2016 to june 30, 2017 total participants served were 451; nchp provided $194,421. 12 in temporary financial assistance for rents, security deposit, utilities, utility deposits, moving expenses, emergency and general housing assistance, and car repairs to veterans families.
from july 1, 2016 to june 30, 2017, national community health partners completes its third year of its fourth cycle to provide capacity building assistance for cbos (category b) and also began its fourth year. From july 1, 2016 june 30, 2017, national community health partners provided 48 trainings for yr3 and 17 of the 58 trainings required for yr4 of the ps14-1403 contract. Of the 17 trainings delivered for yr4 from april 1, 2017 to june 30, 2017 twelve (11) were in the 12-cities, one (6) were delivered in the cdc-designated high-impact prevention urban areas. Essentially, this indicates the required in-person trainings have been delivered to the areas with the greatest number of persons living with diagnosed hiv infection. During this time, nchp maintained qualified persons for an hiv-positive (poz) advisory board and the transgender and non-gender conforming advisory board to integrate community feedback and cultural competence. Nchp project director, staff, and the advisory board have developed and/or revamped 12 training topics to hip ensuring that the trainings delivered have the greatest possible impact from july 1, 2016 june 30, 2017.
