Programs: Hospice & community care is dedicated to providing care and comfort to help patients and families live better at end of life. During 2014, hospice & community care provided almost 154,000 patient days of hospice care to patients and their families and served 2,914 patients during the year, regardless of the patient's ability to pay. Eight percent of the patient days of hospice care were charitable care. Physicians completed almost 14,000 hospice and palliative care visits to patients in their private homes, nursing homes, hospitals, physician clinic and hospice inpatient unit. The bereavement program supported 8,631 individuals during 2014 through 239 support groups, over 34,000 pieces of mail, over 4,000 telephone calls and over 775 individual visits.
