Mission: Heights & Hill was formed in 1971 as a collaborative effort between local churches, synagogues, service organizations and interested community members to address the needs of an increasingly frail elderly population living in our community. Our goal is to help our senior citizen neighbors, particularly those without family supports, to live safe and independent lives and to delay or avoid costly and impersonal institutionalization. Toward that goal, we provide a combination of social services, advocacy services, and community building activities. Direct services include social work services (case management), caregiver support services, transportation, education programs, and assistance in obtaining home care.
Target demographics: seniors, their family members and caregiver
Direct beneficiaries per year: over 3,000 older adults and the families and friends who care for them throughout 19 neighborhoods, through the provision of case management, caregiver services and transportation programs.
Geographic areas served: Brooklyn
Programs: case management services to homebound older adults, caregiver services and support groups for the neighbors, friends or family members caring for the older adult in their lives; transportation; Friendly Visiting and volunteer assistance.
