Mission: Heartland human care services, inc. 's mission aims to achieve freedom from want, fear and injustice for people marginalized by poverty, displacement or situations of vulnerability by developing a continuum of services that meet basic needs, build strengths, safeguard human rights and provide positive opportunities for change.
Programs: The hhcs unaccompanied children initiative provides a culturally competent continuum of care from group homes to a therapeutic residential environment for unaccompanied immigrant children who need to be reunited with families or offered legal protections. In addition, the department provides rehabilitative residential alternatives to juvenile detention in cook county and provides transitional housing to homeless youth, ages 18-24. In fiscal year 2017, over 2,700 individuals were housed in protective residential settings.
hhcs familyworks program is a collaboration with the chicago housing authority to deliver comprehensive case management services to public housing residents through a team approach consisting of housing, employment, adult education, clinical, and other needed services. Familyworks serves all members of the household including youth and seniors. The team carries out a number of activities to positively engage public housing residents in services that lead to greater self-sufficiency starting with an initial assessment of housing and employment needs and barriers. The assessment leads to the development of individual service plans. These plans detail employment housing, support and clinical service needs as well as family needs that encompass the children's positive development. The team also works with families on lease issues and develops supports that help individuals meet work requirements. In fiscal year 2017, over 4,0000 individuals and families were served through the familyworks program of hhcs.
building stable communities (bsc) provides supportive housing for families and individuals in their transition from homelessness to permanent housing. Using a strengths-based family case management model, bsc provides intensive case management, housing location, rental assistance, financial literacy, child development, parenting support, job readiness and placement, life-skills development, and follow-up services. Bsc housing consists of scattered-site transitional housing. In fiscal year 2017, bsc served approximately 240 households, which included over 550 adults and children.
