Good shepherd hospice is a medicare-certified agency that provides end-of-life care in polk, highlands and hardee counties in florida. In 2017, good shepherd hospice served 3,795 patients in its communities. Good shepherd hospice maintains 53% market share in polk highlands and hardee counties in 2017. These services are provided by interdisciplinary teams composed of qualified professionals and trained volunteers. Good shepherd hospice also has developed an extensive network of partnerships with hospitals, assisted living facilities and nursing homes to identify patients who would benefit from the services available.
