Mission: Easterseals michigan works to change the way the world defines and views disability by making profound, positive differences in people's lives every day. For nearly 100 years we have been serving the residents of michigan. During the year we have served and supported more than 12,000 children and adults with disabilities and their families. As the world grew, so did esm and today our mission serves people of all ages with many different disabilities. Our specialties include behavioral health, substance use, trauma services, early intervention, medical rehabilitation and services to those with autism.
Programs: Adult services - esm provides community and office based therapeutic and rehabilitative evidenced based services and supports to adult individuals who have been diagnosed with a mental health, substance use and/or co-occurring condition(s). Esm served 7,591 adults in the past year. Program locations include older adults in kent county, southfield adults, pontiac adults and macomb centerline, agrability for farmers. Esm operates a number of programs with a diverse funding structure in each of these locations. Timeliness outcomes: request to first service within 14 days (benchmark = 95%): 95%intake to ongoing service within 14 days (benchmark = 95%): 99%continuity of care post discharge from hospital (benchmark = 95%): 87%recidivism: % of re-admits to psych hospitalization within 30 days (benchmark = 15%): 11. 7%

Esm served 2,897 children and families in the last year. Program locations include oakland county - southfield and auburn hills family services, waterford, kent county homebased/case management, genesee county and macomb centerline. Esm operates a number of programs with a diverse funding structure in each of these locations. early intervention, children's therapy, medical rehab services, autism, and trauma assessments and planning - esm provides early intervention diagnostic and therapy services, infant mental health, and medical rehabilitation services to children of all ages. These services include the p. L. A. Y. Project, infant mental health, incredible years, applied behavioral analysis (aba), occupational, and speech and language therapy and miracle league of michigan. Esm served 2,459 children and families in the last year. Program locations include kent county, waterford, flint, auburn hills, macomb - centerline and southfield. Programs include: infant mental health, developmental assessments, trauma screening and neurodevelopmental assessments, home based therapy, parent child psychotherapy, applied behavior analysis, ot, speech and language therapies, miracle league of michigan, and incredible years parenting and early intervention programs. Remarkable outcomes - provided 1747 trauma screens to michigan children in fy17; 41 trauma assessments and 351 neurodevelopmental multidisciplinary trauma assessments to children exposed to trauma.
