Programs: Information and Referral: Clients using Information & Referral Services call 2-1-1 and speak with a community connector who is trained to understand and carefully attend to any trauma and individual circumstances they may be going through. Based on what they share with us, each client may choose to connect with the resources that we identify using a resource database that we manage. Safety Net Program: Clients using 211 San Diego’s Safety Net services may engage, retain, and enroll in different programs designed to reduce poverty. By using this unique, phone-based program, clients may eliminate some of the barriers they face to in-person enrollment, such as transportation, childcare or physical limitations. Care Coordination: Clients presenting complicated barriers to accessing housing or healthcare may receive a specialized referral to this program where they will set goals to overcome their individual barriers and access a multitude of necessary services. Community Information Exchange: Nonprofit, government, and healthcare providers may join the CIE partner network to become part of an ecosystem comprised of digitally connected multidisciplinary network partners that use a shared language, a resource database, and an integrated technology platform to deliver enhanced community care planning. Data Reporting: The 211 Resource Database contains over 6,000 services across multiple service sectors. When we combine resource with data about community needs, we can create asset maps at the neighborhood or regional level and identify potential service gaps. We also review client data in aggregate and often in partnership with local research institutions, to produce reports which we share with the community to build a picture what our community needs and to contribute to predictive analytics that may lead to new recommendations for policy and resource development. Disaster Response: 211 San Diego is the region’s 24-hour non-emergency information line. During or following a disaster, including public health emergencies, people may call 2-1-1 for critical information, such as road closures, evacuation routes, food assistance programs, shelters, testing/vaccination centers, and more.
