The mission of mental health resources is to foster hope, health and recovery for those affected by mental illness.
In july 2014, mental health resources' metro team completed a three-year project in partnership with medica. Through a three-year grant with the medica foundation, the metro team provided intensive community based services for individuals experiencing elevated symptoms of their mental illness. The services are provided in the community and individualized with short-term strategic interventions intended to stabilize patients and reduce unnecessary psychiatric hospitalizations due to stressors, barriers and acute psychiatric symptoms. These services are intensive (up to four days per week) and short-term (three to six months). The project demonstrated outstanding results, including an average monthly health care cost reduction of 49. 8%. In 2014, 153 clients were served. Case management services - mhr provides targeted case management (tcm) in ramsey, hennepin and dakota counties. These teams help individuals with serious and persistent mental illness (spmi) gain access to needed medical, housing, social, educational, financial, vocational and other services necessary to meet their mental health needs. In 2014, 2,139 clients were served. Mhr operates assertive community treatment (act) teams in ramsey, hennepin and dakota counties where each team functions as the primary treatment provider for the clients' mental health needs. In 2014, 240 clients were served. Tcm and act teams' clients receive armhs and ils services when eligible. The armhs program offers education and coaching in strengthening basic social and living skills essential to fostering mental health recovery and managing the demands of independent, community-based living. The ils program trains and assists individuals in enhancing their living skills to manage independent community-based living. The mental health outreach clinic (mhoc) is licensed under minnesota rule 29 and provides in-home therapy to mhr clients and their families, and is staffed by psychotherapists and a consulting psychiatrist. In 2014, 176 clients were served. Care coordination - mhr staff members deliver special needs basic care (snbc) coordination for a voluntary managed care program to ucare, metropolitan health plan (mhp), and medica members between 18 and 64 years old who have a mental health, physical health or developmental disability. The majority of services are provided for clients in and the 7-county metropolitan area, but mhr is contracted to provide services for 38 counties throughout minnesota. In 2014, 2,080 clients were served. Housing programs - mhr administers over 800 rental subsidies in the 7- county metro area. The goal is to ensure positive housing outcomes by communicating effectively with clients, service providers and property managers to achieve the best possible result. In 2014, 870 clients were served. The supportive housing program helps people diagnosed with mental illness and/or substance abuse disorders find places to live and be successful tenants. Mhr's housing staff work with several multi-family housing complexes and serves clients in ramsey, hennepin and dakota counties. In 2014, 124 clients were served. Community support program (csp) - the seward csp counselors assist clients who suffer from mental illness living independently in hennepin county by assessing their needs and providing or coordinating services to support the client's ability to remain in the community. The seward csp also runs a drop-in center that provides a wide array of weekly psychosocial groups, day trips/activities, daily healthy snacks and a monthly dinner. In 2014, 408 clients were served.
