Programs: Community health partners is a federally qualified health center providing primary health care services for residents of park and gallatin counties in montana. During fiscal year 2014 - 2015, our four medical clinics located in livingston, bozeman, belgrade, and west yellowstone served 10,250 patients with 33,845 medical and resource assistance visits, while 933 patients were served with 7,190 visits by our integrated behavioral health specialists. Our dental clinics located in livingston and bozeman provided dental services to 3,995 patients through 10,658 visits. Patient fees for all services are adjusted using a sliding fee scale based on federal poverty guidelines. During this period, sliding fee discounts applied to patient visits totaled $2,138,873. Through the medication assistance program, 867 prescriptions were provided at little or not cost to patients, while 8,377 prescriptions were provided through the federal 340(b) program. We estimate that access to these pharmacy programs provided savings of approximately $1,039,395 for our patients during the period july 1, 2014 through june 30, 2015.
chp-learning partners provides integrated educational programs and support services that empower individuals and families to become self-reliant, life-long learners. During the 2014-2015 fiscal year, 57 students were served in the adult basic education classroom, 50 families received intensive case management through the worc program, and 64 families participated in the parents as teachers program. An average of 164 free children's books were distributed to low-income children each month.
