Programs: The clinic provided comprehensive and preventive care through medical, dental, vision, pharmaceutical, and behavioral health services for children, youth, adults and seniors. The clinic also provided enabling services such as benefit enrollment and transportation assistance. Approximately 9,000 patients chose nhan hoa clinic as their medical home for an approximate total of 70,000 encounters during this reporting period. In 2017, the clinic is ranked in the top 30 percentile for quality care by the health resources and services administration.
the health education outreach team provided culturally and linguistically appropriate education on the prevention and management of chronic diseases by planning and executing multiple campaigns to raise awareness. These campaigns included a back-to-school backpack campaign to stress the importance of annual wellness physical exams for children and provide free backpacks and supplies to children who received physical, dental or eye exams. The team also hosted a health fair event that provided free flu shots and free dental, bone density and eye check-ups to raise awareness of preventive care.
