Mission: Working together to improve the quality of life and health of yamhill county care organization members by coordinating effective care.
Programs: Oregon health plan program: through our contracted relationships we provide access for low-income yamhill county, oregon health plan members. Our work involves coordinating quality healthcare for members and contracting with public and private providers to provide healthcare services. Yamhill community care organization is a community organization operated for the benefit of the poor and vulnerable in yamhill county, oregon. As a community organization, we are empowered by the board of directors and stakeholder representatives to direct revenue and community investments into bolstering the healthcare delivery system and making the necessary changes to meet both the legislation and community need. The past year has included staff expansion, allowing for greater programmatic accomplishments. A wellness center now offers pain management and movement therapy, as well as massage therapy for its graduates. The community health worker hub works with high-risk patients to identify health and health-related social needs and address them, and the hub has an outreach program called wellness to learn in mcminnville schools. The third round of community investment grant awards, called build forward, was awarded for the following projects:project one: the capitol dental integrated behavioral-dental healthcare project reaches out to children in schools to deliver screenings and sealants. It targets children and adults with mental health concerns by setting up a teledentistry program in a behavioral health facility. Project two: mcminnville internal medicine's transitional care management and advanced care planning project reduces inpatient and emergency department admission rates by providing transitional and chronic care interventions with its patients. A nurse case manager reaches out to patients with chronic health and/or behavioral health conditions, referring them to additional care or support. Project three: a county-wide oral health and immunization carnival brought together multiple clinics to offer vaccines; oral health screening and education; and, at some clinics, sealant application, on the same night. Providence medical group in newberg is integrating physical therapy into their primary care model by employing a physical therapist to manage patients with musculoskeletal problems and serve as a consult for care teams. The pt's early intervention will allow patients to receive the most appropriate care and reduce unnecessary additional visits, medication use, and imaging services. Project four: provoking hope's responsible moms program builds supports around pregnant women who are struggling with addiction. Responsible moms offers support groups, baby showers, and social connection to women and aims to help them remain clean and sober for the duration of their pregnancy and through child-rearing. Project five: the student nutrition and activity clinic for kids (snack) program delivers personalized healthy lifestyle support to children and families. It connects families with linfield intern mentors, and offers activities and education outside of the clinic. Funding for this program will support snack staff as well as activities like student wellness and games night, which creates a fun, group environment for adolescent well-care checks, as well as community events like a 5k fun-run for all ages. Project six: the west valley fire department's community ems program will expand to employ two paramedics to deliver non-emergent medical services in homes around the county. The paramedics offer screenings, post-operative care, and preventative services to members of the community who have difficulty accessing transportation, are high-frequency emergency department utilizers, or who have been referred to the program. This program reduces ed usage and increases preventative services and education for yamhill cco members. Project seven: willamette valley hospice is working closely with patients who have advanced illnesses and their families, coordinating their care, assisting with advanced planning, and connecting them with hospice services as soon as appropriate. The grant will support recruitment and training of a care team to launch the program. Willamette valley medical center is taking the lead on integrating local clinics' electronic health record systems with the wvmc lab, enabling clinics to receive health records and receive notification about patient visits and lab results from wvmc. Project eight: willamette valley medical center emergency department will be building and staffing a safe environment for emergency department visitors. The behavioral health improvement project will transform an intake room for patients with behavioral health conditions to go when they need a secure place to calm down and feel safe. The project will also hire security guards for the protection of both patients and medical staff and hire social workers to intervene in escalating situations, referring patients to additional resources and care as appropriate.
