Programs: Life (living independence for the elderly) is a comprehensive health care and supportive services program designed to enhance the ability of older adults 55 years of age and older to live safely in their homes and communities. It is a capitated program, funded through the pennsylvania department of human services and centers for medicaid and medicare services. Most life participants are dually eligible for medicare and medicaid and therefore receive the program at no direct cost to them. Life programs are known nationally as pace (program of all inclusive care for the elderly). Life armstrong county is a partnership of lutheran seniorlife and armstrong county memorial hospital. Life coordinates and provides all needed preventive, primary, acute and post-acute care services to its participants through an interdisciplinary team of health care professionals that evaluate, assess and provide for the individual needs of the participant as they change through various stages of aging or medical need. Based on an individual's need, life can provide: comprehensive medical care by an interdisciplinary team of healthcare professionals; an on-call nurse and physician available 24 hours a day; adult day services; personal care; home care; housekeeping and laundry; rehabilitation therapies; transportation to the life center and medical appointments; medications and medication management; meals; medical equipment; and dentistry, optometry, and audiology.
