Mission: To improve the health of the medically under-served population in riverside county, california, by providing preventive and primary health care services and other critical community and social services.
Programs: In 2017, through the generous support of community members, organizations, local & federal governments ucap served over 3,200 homeless individuals for more than 10,800 medical visits in riverside county. About 92% of the visits that occurred were for patients with no insurance, living at or below 100% of the federal poverty level. Urban community action projects (ucap) dba health to hope clinics, believes that the homeless and other under-served individuals have the right to housing, health care and social services that empower and enhance well-being. Addressing the complicated intersection of critical human needs of the under-served and vulnerable communities in riverside county is at the forefront of ucaps service delivery model. Partnering with local and national like-minded organizations and individuals, ucap embraces sustainable, reproducible, community-based solutions to health care that enhances community livability for sheltered and unsheltered homeless families and individuals. Ucap services homeless people where they are, whether it literally be on the street or in a part, an encampment under a freeway by-pass or in a shelter. We take primary care and social services to the homeless via our street medicine program, mobile clinic program, and fixed site clinic program within shelters of riverside county.
