Mission: At the neighborhood hub our mission is to be a beacon of holistic transformation that fosters a healthy african american community through competent navigation services, leadership development and advocacy.
Programs: The hub provides direct services, information, education, referral, assistance, and advocacy to approximately 2,500 individuals each year. Navigators served approximately 550 families each year with families receiving intensive case management and personalized case management on topics such as housing, unemployment, access to transportation, securing financial assistance, accessing health care, and legal advocacy.
the hub continues to make an impact on the community by providing programming through our healthy life initiatives. We receive funding through the minnesota department of health-advancing health equity department and the city of minneapolis-health department. With this funding, we have created a healthy homes team that provides workshop and one-on-one home visits that include resources and education for residents on how to create and maintain a healthy environment. We continue to include our youth in awareness raising campaigns to educate their peers/community on the harm that can be caused by sugary beverages and the dangers of menthol products.
the cultural navigation pilot program began on july 1, 2017. Cultural navigators serve as a bridge between families involved in the child protection system and hennepin county. The project is slated to support families with navigating systems, addressing barriers, and advocating for themselves and their children. The program has successfully served 20 families to date and is on target to meet the goal of servicing 70 families by june 2019.
