Mission: TO SERVE HOMELESS PERSONS WITH HISTORIES OF EMOTIONAL AND/OR MENTAL DISABILITIES BY PROVIDING THEM HOUSING AND RELATED SERVICE.
Target demographics: Adults who have experienced homelessness and have a history of mental illness
Geographic areas served: Over-The-Rhine, Cincinnati, OH
Programs: Permanent housing includes 199 units for homeless individuals with histories of mental illness.
Supportive services includes : medication monitoring, crisis intervention, income assistance, skills building and resource linkage.
Permanent supportive housing includes shelter plus care : a program designed to provide housing and supportive services on a long-term basis for homeless persons with disabilities. Tender mercies maintains 30 shelter plus care subsidies.
outreach - a collaborative program with greater cincinnati behavioral health services in the projects for assistance in transition from homelessness (path) team to identify and engage homeless persons in a therapeutic relationship in order to help them apply for housing. who are in transition from homelessness to assisted living and from assisted living to independent housing. 23 residents were served during the current year.
