Programs: Grandfamily resource center: sown connects at-risk children and the grandparents who are raising them to our comprehensive continuum of education, health, and social services designed especially for these families. Sown provides grandparent support groups, parenting education programs, educational workshops for grandparents and grandchildren, crisis intervention, and individual support to access resources/services.
telephone counseling groups for homebound, older adults and parkinson's care partners: master's level social workers lead groups by telephone conference, providing essential support and "safety net" services (with weekly contact, issues are identified, solutions discussed, and services accessed before a crisis occurs). The service is portable, enabling seamless service provision across settings (home, hospital, recuperating with a relative, or rehab).
community-based support groups for older adults: groups meet weekly in a variety of community locations such as senior centers, apartment buildings, assisted living facilities and community centers, providing emotional support and health education.
