Causes: Community Clinics, Health
Mission: The mission of Summit Community Care Clinic is to provide exceptional, integrated, patient-centered health care services designed to meet the needs of people who experience barriers to accessing care, regardless of their ability to pay.
Results: In August of 2012, SCCC was awarded Federally Qualified Health Center - Look Alike status. This means that SCCC can offer services to more of our medically underserved neighbors and friends, including those patients with Medicaid and Medicare. This also creates more financial sustainability for SCCC, and is our state and federal government's commitment to serving those who need it most. In the last year, SCCC also implemented an electronic medical record, which allows us to better care for our patients. In October of 2012, SCCC implemented a new "Health Educator" program. The role of the Health Educator is to work alongside the medical provider and offer education, coaching, support and follow-up with patients who want to take control of their own health. This allows the patient to spend time with a health care professional (the Health Educator) and get their needs met, while allowing the provider to see other patients. This is a cost effective and intuitive model, and we predict this will become the future of health care.
Target demographics: Medically underserved members of Summit County, CO and surrounding communities. Our target population is the low-income, uninsured or underinsured who have no other options for health care. We also serve patients with Medicaid and Medicare.
Direct beneficiaries per year: In 2011, SCCC served 5,220 unduplicated patients.
Geographic areas served: Summit, Lake, Eagle, Grand, Park, and Clear Creek Counties.
Programs: SCCC provides comprehensive medical, mental health and oral health care in one location. Programs include: primary and preventative medical care; chronic disease management; physical therapy; reproductive health and family planning services; community health screening events; individual mental health appointments and medication evaluations; drug and alcohol screenings and substance abuse treatment; oral health maintenance and treatment; acupuncutre and nutritional counseling; and patient navigation services. In 2011, SCCC saw 5,220 unduplicated patients for a total of 14,088 visits.
