Mission: The mission of southwestern pennsylvania area agency on aging, inc. Is to promote the well-being of older adults residing in fayette, greene and washington counties through a planned, coordinated, collaborative approach to health and human services to assist seniors with remaining in their homes and communities for as long as possible.
Programs: Aging block grant funds as part of our ongoing commitment to providing high quality services to older adults and caregivers in the community, we are launching a new quality assurance and performance improvement program. Our quality assurance and performance improvement program includes an annual needs assessment of individuals served, logic models, evidence and professional standards to guide the implementations of our programs and services, key performance indicators to measure what matters most, and tools to support our leadership and staff in monitoring performance. Southwestern pennsylvania area agency on aging is adopting the quality assurance and performance improvement program and supporting tools to identify opportunities for improvement, to consistently enhance our programs to best serve older adults and caregivers, and to demonstrate our impact.
medicaid - aging waiver as pennsylvania is moving from medicaid fee-for-service to a managed care model, the southwestern pennsylvania area agency on aging is providing training to staff in an effort to prepare to work with managed care organizations and other health payers to support person-centered, coordinated and cost-effective long-term services and supports care for older adults and persons with disabilities.
