Causes: Health, Nursing Facilities, Seniors
Mission: To improve the health and wellness of the elderly, indigent and medically needy in the southeastern united states through a wellness focused continuum of care that treats the medical needs and provides education to the elderly and indigent on how to live healthier and happier lives.
Programs: See schedule oin accordance with our long term plans to provide a comprehensive health and wellness program that addresses the needs of each community we serve, solaris entered the skilled nursing and assisted living markets. Solaris provided multiple services to further the mission of our organization in 2016. These services included:1. Skilled nursing and assisted living2. Consulting services to skilled nursing facilities and assisted living facilities. Our services identified opportunities to improve systems that ultimately improve patient care and provide additional resources for the facilities to use to improve the quality of life of residents. 3. Participated on community committees to identify opportunities to reduce hospital readmission rates. 4. Participated on community committees to identify the root causes of falls in the elderly in their home setting. 5. The foundation's long-term goal is to improve the overall quality of life of individuals throughout the southeastern united states through a comprehensive health and wellness program that addresses the needs of each community we serve. Through our group of affiliated entities and a network of healthcare providers, we will develop a wellness-based continuum of services that addresses the medical, social and wellness needs of the elderly, indigent and children in each of the communities we serve.
