Emergency meals on wheelsthe emow program is designed to meet the needs of indian river county seniors in nutritional crisis providing home delivered meals for isolated, home bound seniors over age 60 who have been discharged from a hospital, rehabilitation facility or nursing home within the last 30 days or, who have dependence on a caregiver who is entering a hospital or rehab center. The program goals are to prevent hospital or facility readmission, facilitate recovery, and defer nursing home placement, 226 unduplicated homebound seniors received emergency meals on wheels, 5,816 meals served.
case management:case managers work with seniors (and their caregivers) to identify and arrange for the services needed to support the individual in their home. In home support includes, companionship, homemaking, personal care, meals and respite. 304 older adults received in-home services; 100% of case managed clients were deferred from nursing home placement. 127 households were assisted through the emergency home energy assistance for the elderly program. Information and referral for programs and services are available to seniors. Approximately 1,536 individuals received assistance through information and referral.
