Mission: Selfhelp special family home care, inc. , d. B. A. Selfhelp family home care is a not-for-profit organization dedicated to maintaining the independence and dignity of seniors and at-risk populations and will lead in applying new methods and technologies to address changing needs of its community. The goal of this home care program is to promote independence through the coordination of integrated health services.
Programs: Selfhelp family home care is a certified home health agency which provides comprehensive home-based care. Its interdisciplinary services, which are primarily offered in patients' homes, include a combination of nursing, social work, and specialized disciplines. In 2014 the chha received general purpose designation allowing it to meet the broader needs of the community. Specific services include: 1. Experienced home care nurses 2. Treatment adherence intervention; 3. Behavioral health assessement and intervention by nurses; 4. Physical and occupational therapies; 5. Speech/language pathology; 6. Homehealth aide services and personal care; 7. Social work services for permanency planning, community linkage, and support; 8. A nutritionist for diet assessment and education; 9. Medication adherence programs; 10. Disease management programs; 11. Wound care; 12. Fuzeon injections; and, 13. Telemedicine. In 2016, the organization provided 465 hha visits, 1,162 nursing visits, and 734 therapy visits to clients.
