Causes: In-Home Assistance
Mission: Seasons home care seeks to identify and provide quality in-home services that allows area adults age 18 and older to maintain their independence in their activities of daily living with the help of appropriate support systems that address problems affecting their quality of life.
Programs: Seasons home care intends to provide educational support through the development of a resource, information, and referral service that will assist clients and families as they navigate the decision process for long term care that does not involve institutional care, or at least delays the ultimate decision. Our goals are:-to assess the client, identify their specific needs, and then provide the appropriate services to maintain individual safely in their environment of choice. -to assess the client's financial resources and subsequent eligibility for entitlement. - to administer title iii (home care community block grant funds) effectively and in compliance with applicable regulations. - to provide in-home personal care aide services using non-certified and certified nursing assistants (cna's) to manage household and personal care activities, as appropriate. - to provide an information and referral service to assist clients in researching needed services not provided by seasons home care. - to facilitate a multiple agency approach that allows all community services and seasons home care staff to utilize existing community resources in serving clients. - to identify needed services not available in the county, report this information to the board and assist in developing the needed services.
