Programs: The schaffer extended care center at montefiore new rochelle hospital is part of montefiore health system, the university hospital system for albert einstein college of medicine, a 150-bed comprehensive skilled nursing facility offering short-term rehabilitation/sub-acute care and skilled long-term care, as well as an adult day services program that provides a wide range of healthcare and social services for medically frail seniors and those with chronic conditions or disabilities. Schaffer's rehabilitation program has been helping cardiopulmonary, orthopaedic, and surgery patients recovery so that these patients can return to their homes and families as quickly as possible. The short-term program specializes in cardiopulmonary rehabilitation, post-surgery and stroke recovery, as well as fracture, fall and frail patient care. Schaffer extended care center began operations on november 6, 2013 at the site of the former howe avenue nursing home in new rochelle, new york. Schaffer extended care center provided 51,589 days of skilled nursing and residential health care during calendar year 2016. The center maintained an average daily census of 141 and operated at an 94% occupancy. Schaffer extended care center also operates a licensed outpatient adult day care program for non-residents of the care center. The center provided 6,832 adult day care program visits in calendar year 2016.
