Programs: Scenic bluffs, recognized as a level-two patient-centered medical home (pcmh) by the national committee for quality assurance, provides primary, preventive healthcare to the underserved populations of monroe, vernon, crawford and la crosse counties in western wisconsin. From 2016-2017, 7,561 patients were served, represented by a total of 23,090 appointments. Of these appointments, 27. 9% were medical in nature, 61% were dental appointments, 5. 6% were chiropractic appointments, 5. 5% were behavioral health, and many patients received health education and pharmacy services. Scenic bluffs aims to address social determinants of health through educational, prevention-based programming that connects patients with resources and education. This is possible through our health educators, help team of certified application counselors, and the nearly 50 coalitions, groups and boards scenic bluffs participates in.
