Mission: The svila collaborative d. B. A. Centro svila is a treatment program devoted to the recovery and healing of individuals, families, and communities suffering from emotional and psychological distress. Our vision is a healthy, engaged and equitable community. Our mission is providing access to quality healthcare and eliminating health disparities through prevention, education, and collaboration. Our comprehensive model of care works to decrease health disparities in bernalillo county by providing: outpatient mental health services, systems navigation, no-cost health insurance enrollment assistance for medicaid and the new mexico health insurance exchange, school-based counseling, therapeutic gardening, food security support, drug and alcohol counseling and recovery support, and supervision, training and research opportunities for students and health professionals. Programs: Clinical: the svila collaborative provides preventative primary mental healthcare direct clinical services including: outpatient counseling, psychotherapy, and drug and alcohol addiction recovery treatment available to anyone in bernalillo county and the surrounding community regardless of their ability to pay. During fy 2014, the svila collaborative: contracted to provide off-site school-based clinical services at robert f. Kennedy charter high school; provided healthcare to over 800 unduplicated patients; hired one new clinical provider; implemented a psychoeducational and peer support group for adolescents with type 1 diabetes and a parental support group; provided facilitation for the girasol program that provides education and counseling for patients diagnosed with type ii diabetes and pre-diabetes and support for their families; provided outpatient drug and alcohol counseling to over 50 court-mandated patients; and expanded services in our therapeutic garden.
outreach and enrollment: during fy 2014, the svila collaborative worked to expand health insurance coverage in new mexico by training employees to become enrollers for presumptive eligibility medicaid on site application assistance (pemosaa) and the new mexico health insurance exchange. Centro svila hired a team of 8 bilingual outreach and enrollment specialist who scheduled over 1200 appointments and successfully enrolled 534 people in medicaid and 360 in private health insurance. The enrollment team also participated in outreach efforts to inform the public about health insurance options and eligibility. The outreach and enrollment team performed over 930 hours on outreach and education efforts. The enrollment team attended over 190 events within the bernalillo area, focusing our outreach and education efforts to target the south valley communities. We encountered over 15,000 individuals by distributing marketing materials within the community and school events, local businesses and community centers. Our bicultural/bilingual team was able to reach out to latino/hispanic and spanish speaking new mexicans who are the largest uninsured population in nm.
case management: the svila collaborative provides healthcare navigation and case management services to clients that have difficulty accessing services to meet their basic needs. During fy 2014, the svila collaborative participated in the pathways to a healthy bernalillo county program providing services to some of the most economically, psychologically and physically vulnerable residents of bernalillo county. In addition, the svila collaborative coordinated student volunteers and interns from unm, nmhu, and nmsu to provide case management services for our patients. The svila collaborative expanded our community health worker (chw)/promotora efforts to integrate research, education and services to reduce behavioral health disparities with the hispanic and latino populations. Chw training was integrated into our heath insurance outreach and enrollment efforts to facilitate newly insured in finding a healthcare home and other social supports. The svila collaborative also received funding for the community partnership for health equity program that focuses on upstream healthcare prevention, community engagement, and integration of healthcare and social services. This program allowed the svila collaborative to serve as the fiscal agent and liaison for the girasol and access programs. The girasol program provides one on one educational sessions for spanish-speaking patients diagnosed with diabetes and pre-diabetes to help patients better understand and manage their blood sugar through diet, medication management and exercise. The girasol program works in collaboration with primary care physicians and mental health providers at centro svila to provide better support for patients managing chronic disease and their families. The access program supports after school activities for spanish-speaking elementary, middle school and high school students in the south valley. The program combines community engagement, direct services and health promotion through chws. The access program formed its own soccer league and provided free after school education, athletic, cultural, art and science opportunities for south valley children and their families.
