Programs: Rise wisconsin, inc. Provided comprehensive case management, family advocacy, and wraparound services to 210 children and adolescents with mental illness under a specialty medicaid managed care program. In addition to serving 210 children in 2017, rise wisconsin, inc. Provided transitional case management services to 69 young adults between the ages of 18-30 who are living with a mental health diagnosis.
in 2017, the agency continued the early childhood initiative, which is a voluntary home visiting program in madison/dane county, wisconsin, for pregnant women and families with children aged 0-4. Early childhood initiative services are grounded in the knowledge that families are more successful when parents and children have access to responsive, comprehensive and community-based support. During 2017, rise wisconsin, inc. Worked with 121 families through the early childhood initiative program.
the court ordered evaluation program partners with mental health professionals to conduct court ordered evaluations for individuals under the following wisconsin statutes: juvenile justice code 938. 295, children's code 48. 295, emergency detention 51. 15, involuntary commitment for treatment 51. 20, and guardianship 54/55. In 2017, the agency brokered 945 evaluations for individuals in dane county under these state statutes.
