Programs: Employment program:employment programs are designed to provide individuals with long-term support. Through the community supported employment program, job support is provided to persons employed individually in the community (called individual placement), or to persons placed in community group settings with fellow proact, inc. Employees (called enclave or work crew placement). Ongoing assistance is provided so that successful employment is maintained. Through the center based employment program, individuals are employed in the on-site integrated production facilities. Continuous support is provided from staff, as individuals master industrial skills. Employment can be long or short term for those working toward placement in the community supported employment program.
day training and habilitation programthe day training and habilitation program provides consumers with long-term supported employment and lifeskills training. Through the center-based employment program, consumers are employed on site with packaging and assembly work. Continuous support, guidance, and training are provided from staff to assist consumers to enhance their level of independence and master of work skills. Through community-supported employment (called enclaves), which completes specific job tasks to maintain a successful partnership with community businesses, consumers are provided ongoing staff support to ensure successful employment. Through lifeskills training, consumers choose from an array of classes to increase survival skills, promote self-awareness, and develop life long resources. Services offered include: prevocational training, lifeskills education, recreation therapy, occupational therapy, speech therapy, senior option programs, community integration, medication administration, behavior management and medical consultation, and cash management. Traumatic brain inury (tbi)professional therapy services=speech and occupational therapy=physical and psychological therapy=structured exercise therapyquality of life=community orientation and integration=social events=group experiences=enhanced daily living and memorysupported employment - support services designed to assist consumers to attain and maintain successful employment in community, individual, or group settings. Pre-vocational services - center based activity - designed to assist an individual to meet employment goals through training and staff support in an industrial packaging and assembly setting. Behavior aid - a one-to-one staff client ratio is available for an additional per diem charge for those persons who require increased supervision, assistance and behavioral considerations. Proactive partners - an employer-of-record service supporting families that hire assistance such as respite care. A county arranges for appropriate funding and proact functions as the payroll agent.
vocational programs:the following are time-limited programs used to assist individuals in securing and maintaining employment:assessment/work evaluation-a program designed to identify an individual's employment skills and abilities so that vocational potentials are reached. Work skills and behaviors are assessed by using real work experience. Work adjustment training-an individualized program used to assist in the development of effective work skills and habits (such as attendance, punctuality, organization, productivity and on-task concentration) through on-the-job training and staff support. Job development and placement-a service designed to assist in securing employment that meets the individual employment goals. Some of the elements include job search, resume creation, interview preparation and follow-up. Job coaching-individualized one-to-one training provided as needed to help individuals understand and successfully perform assigned job duties. School to work transition-students learn about career options and employer expectations through a variety of real job experiences. Public assistance to self-sufficiency (pass - our welfare to work program-on the job training program (ojt) - a concise two-week on-the-job work evaluation/assessment focused on understanding the individual's over-all employment skills. Center-based or community based - minimum wage employment (allowing the participant to develop the employment and workplace social skills necessary to make successful transition to competitive employment). Job search and placement - to assist in securing employment that meets the individual employment goals, including job search, resume creation, interview preparation and follow-up. Follow-up and employment coaching - one-to-one training provided as needed to help individuals understand and successfully perform assigned job duties. Proact, inc.
