senior services case managementtwo hundred ninety-four seniors received case management services, which equates to over 1,520 hours of services. The goal was 250 clients with 1,000 hours. Senior caf. Ppsc senior caf serves seniors at our 211 novak location. In 2016-2017 we served 10,773 meals, far exceeding the goal of 9,610; a 112% increase. Home delivered mealsppsc home delivered meals, or meals on wheels, delivered 32,162 hot meals, a 123% increase over the expected 26,150. A team of 126 volunteers helps prepare and deliver these meals 365 days a year. Adult day respitethree thousand eight hundred and fifty-two hours programming served 22 new clients. We did see a drop in the number of community members accessing this service for the first time in many years. Depression preventionas part of the county-wide collaborative known as the older adult collaborative (oac), ppsc far exceeded our goal of 275 seniors for depression screening; 349 seniors were screened, all were referred to additional services, 39 received additional education about depression. Caregivers support groupeight unduplicated family members participated in our support group. Our current funding was set for 6 family members. Involved elders programover 13,448 hours of educational instruction were provided through our partnership with the srjc involved elders program. Iride petalumain its second year, we continue to see steady growth, adding about three to four new rides a month. Monthly, we provided on average 241 one way rides and at least 2,500 miles driven. Over 190 mobility management hours were provided. Fallproof! A new program for seniors, in partnership with the petaluma senior center, we have 30 seniors who participate weekly in this evidence-based fall prevention program.
