Mission: Partners in care's mission is to provide end of life care that honors the dignity and needs of individuals and families and sustains an organization that enriches the lives of patients, families, its teams and its regional communities.
Programs: The organization provided home and inpatient hospice care, palliative care visits, home health care, outpatient therapy visits, and transitional care for the central oregon region. The organization served over 1,033 hospice patients at the organization's in-patient facility, patient homes or in other facilities. The organizations mds, rn's, and sws provided 500 palliative care visits to patients not enrolled in either hospice or home health. The organization's home health caregivers served over 1,191 patients and the outpatient therapy program provided 126 visits. The organization received 1,069 referrals into the transitions program, 51% of whom were ultimately referred to pics other programs.
