Causes: Family Services, Human Services

Direct beneficiaries per year: over 150 individuals dealing with serious, chronic disabilities and their families gain peace of mind about the future

Programs: Life Planning, Family Surrogacy, Recreation, Health and Wellness, and Cognitive Enhancement Therapy (CET)

PLAN helps people plan for the future if they or a family member has a disability and then acts like a family surrogate for them long term. They make people feel included and have been a god send for me and many like me who struggle with mental illnesses and other disabilities. I feel better about my future knowing they will be there when my mom passes away. They have helped her set up financial plans that will help me in the future without throwing me off benefits too. They have recreational opportunities and treat me like a someone in a family. I've known them for years and they really have helped me in my life and will continue to as I age.
