Causes: Financial Counseling, Human Services, Transportation Assistance
Mission: To connect individuals in the Bay Area with the services they need to maintain or improve their independence and dignity. We do this with a holistic approach, applying technology and partnerships to achieve the highest benefit for our clients and community.
Programs: OUTREACH programs: Paratransit Program offers door-to-door, wheel-chair accessible service to more than 13,000 residents of the County whose disabilities prevent their use of other public, fixed-route transportation (like buses or rail) or the use of their private automobile. Clients use the service to get to medical appointments, nutrition programs, senior or adult day care centers, for life-sustaining treatments such as dialysis or chemotherapy, or to travel to schools or jobs. Our disabled transportation clients range in ages from as young as 2 years to over 100 years. The Guaranteed Ride Program - provides free transportation for welfare participants engaged in employment-related activities. The program supports participant's efforts toward self-sufficiency by providing transportation to school, training, job interviews or work. As part of the program's benefits, participants are able to use the service to drop-off their children at school or daycare on their way to their training or work related activity and can also use the service to pickup their children on their way home at the end of the day. This program has provided over 140,000 passenger rides to work, day care centers and schools and enrolled over 6,600 low-income participants, mostly women, have enrolled. Give Kids a LIFT ? After-School Transportation for Children - provides free after-school transportation for children of low-income families. Transportation services are provided from school to day-care, after-school programs, neighborhood and community programs, and other supervised locations as selected by the family. The program is designed to assist parents who would otherwise have to interrupt their school, training or work, in order to transport their children to a safe location at the end of the school day. Children aged 5 to 13 years from CalWORKs and low-income families are eligible. Rides are provided Mondays through Fridays, and flexible schedules are accommodated. In order to ensure the health and safety of these young children, a driver and attendant are assigned to all vehicles so that the children are never left unattended and so that there is always an adult available who can accompany the children to the door of their destination. This project is made possible by the County of Santa Clara Social Service Agency, the Metropolitan Transportation Commission (MTC), and the Valley Transportation Authority (VTA). Senior Outreach Program: provides "hands-on" casemanagement and benefits counseling services to frail seniors and isolated and disabled adults needing help obtaining available services. Trained and experienced case managers work with over 2,000 residents annually, providing personalized assistance with the completion of benefit application forms, securing eligibility documents, monitoring submitted applications, arranging for medical or in-home care, providing money management assistance or other services as needed., Senior Transportation Program: provides free transportation for seniors to basic needs destinations (e.g., medical/health related appointments, nutrition programs, benefit providers, etc.). The goal of the program is to help seniors maintain their independence and self-sufficiency.
