Mission: One stop's mission is to enable seniors to live safely, independently and with dignity in their homes for as long as possible.
Programs: The one stop care management program provides in-home care management to vulnerable and frail seniors with complex needs. Services include all benefits and entitlements screening and assistance as well as care coordination (health related, housing, financial, etc. ) and ongoing counseling, monitoring and re-assessment to ensure clients'safety, maximum independence, and well-being. One stop conducts vigorous outreach and education throughout the community to provide information and assistance about services and all resources for seniors living in the community.
