Mission: To maintain and develop health care models for the well-being and dignity of the elderly and chronically ill through education, advocacy, innovation in services and financing, while advocating for quality and affordable care.
Results: For 42 years On Lok has been committed to meeting the health care needs of the frail elderly in the Bay Area. Our unique approach serves older adults residing in San Francisco, and many areas of Fremont and San Jose, who are 55 or older and experience multiple medical problems that inhibit their ability to live unassisted. Over 1,300 seniors served annually. Upon joining On Lok Lifeways seniors often get a new lease on life, because they are relieved many of the challenges of day-to-day life and the gaps of fragmented care. As an On Lok Lifeways participant said it, “Before I joined On Lok, I was missing doctor’s appointments because I would forget or would not feel well to travel on my own. Now, On Lok’s staff keeps track of my appointments. As a senior, sometimes you feel slow in the morning, but an On Lok driver would give me a call and ask to get ready. So I get up and get ready, and then they take care of me, at On Lok.”
