Mission: Under a contract with the centers for medicare and medicaid services as end stage renal disease (esrd) network 16, the network provides services to esrd patients and providers in alaska, idaho, montana, oregon and washington. Activities are conducted in the areas of quality improvement projects, community education and patient engagement, technical assistance to providers to enhance quality of care and outcomes for patients, investigation of patient complaints and grievances, administrative reporting, data collection, and collaboration with other organizations serving this population.
Programs: Quality improvement: cms requires rigorous work in quality improvement to ensure a safe experience for dialysis patients. Qias include focused work on vaccinations, bloodstream infections, long term catheter reduction and qip improvements. Cms requires that the network identify one innovation project each year and successfully show improvement in this area.
data management: crownweb is the data system used in dialysis facilities. Crownweb and nhsn represent the data systems used within the network 16 contract to track patient activity and infection rates. Nhsn is a cdc data management system that tracks clinical information. Northwest renal network oversees and coordinates this data collection with the facilities.
patient services: this category represents the activities designated by cms that focus on patient and family engagement and patient experience of care. Northwest renal network utilizes their patient advisory council and patient learning and action network to implement meaningful activities in this area.
