Mission: The mission of the north by northeast community health center is to advance health equity and improve health outcomes in a medically under-served community by offering health screening and basic medical services at no cost. Priority is given to african americans and low-income neighborhood residents of portland, oregon.
Programs: Medical services provided in a clinic setting at no charge to medicaid- covered and uninsured adults residing in portland, oregon - 2,103 patient visits serving 412 patients.
community health worker (chw) program provides chw support to some patients. Chw support includes goal setting, identification of barriers to health improvement, home visits, phone calls and health monitoring. Chw program also includes outreach, health education and health promotion in the community.
vegetable and recreational garden provides opportunities for education about healthy eating, nutrition, physical activity and gardening to north by northeast patients. The garden provides fresh produce during the spring, summer and fall months to approximately 412 patients seen during evening clinics.
