Mission: Mission Hospice & Home Care is a Medicare-certified, hospice- and home care-licensed, nonprofit organization that provides physical, emotional, social, and spiritual support for terminally ill patients and their families. We serve San Mateo County and surrounding communities, and we seek to educate professionals, other caregivers and the public about the important role of hospice care. Founded in 1979, Mission Hospice is a local, independent hospice structured to meet patients’ personal needs. We’re committed to providing quality care, with a culturally sensitive and individualized approach.

Programs: Mission Hospice & Home Care provides quality, compassionate end-of-life care for patients and families in the San Mateo County and northern Santa Clara County areas. We help patients live life to the fullest, providing medical, emotional, and spiritual support for those with life-limiting illnesses. Hospice provides compassionate palliative care, emotional, and spiritual support for patients in the last six months of life. Advanced Care provides skilled nursing care and medical social work for homebound patients with life-limiting illnesses. Transitions provides nursing consultation, social work, and volunteer help for patients who need care management, emotional support, and practical assistance.
