Mission: Mnraaa helps older adults thrive in southwest mn by enabling older adults to maintain the lifestyle of their choice. Mnraaa strives to ensure that services are available for older adults and caregivers, by providing information and assistance.
Programs: Mnraaa funds core services to help older adults and caregivers living at home; collaborates with communities, counties and health and social service providers to increase local capacity to meet the needs of an aging population; and provides older adults, their families and caregivers with information about services, financial assistance and health care coverage through the senior linkage line. Persons age 60 and above and caregivers are served. As part of their programs, mnraaa runs the return to community initiative, which is the in-person assistance role of the senior linkage line service that provides in-person care transitions support. Staff work closely with nursing home residents, their families and nursing facility staff to make sure support is in place for the resident's return home. The initiative was expanded to include people living in the community who were at risk of nursing home placement.
