Mission: Operating as mjhs home care, is a not-for-profit, certified home health agency (chha), commited to providing skilled nursing and rehabilitation services for individuals recuperating from an acute epsiode, often elderly and at-risk members of the community. With roots dating back to 1907, when the four brooklyn ladies founded mjhs, based on the core values of compassion, dignity and respect, mjhs home care is committed to providing skilled, caring services to people of all religious, cultural and economic backgrounds in the familiar and comfortable surrounding of their own homes.
Programs: Operating under the name mjhs home care, and with roots dating back to 1907 when the organization was first established by the four brooklyn ladies, mjhs home care is a not-for-profit chha that is committed to serving the needs of individuals recuperating from an acute episode as well as the elderly. Mjhs home care has the experienced staff and clinical programs required to deliver quality home-based skilled nursing care, physical therapy and rehabilitations so that our patients can receive the care they need while remaining safely at home. In 2016, mjhs home care provided 278,384 hours of hha services and 277,144 visits for other related services.
mjhs care management options (cmo) provides care management services. In 2016 cmo provided 32,366 member months of care management services to homefirst members.
