Causes: Health, Public Health
Mission: Our MISSION is to improve local access to wellness education, prevention services and quality healthcare through creative solutions in collaboration with our community. Our VISION is optimal health and wellness necessary to promote quality of life for everyone in our community. The CORE VALUES of Mendonoma Health Alliance include person-focused decisions, integrity, collaboration, excellence, accountability and stewardship. These values guide our actions as we work together to provide healthcare services to our community.
Target demographics: We embrace the whole community, convening valuable resources from our partners, and striving to be a "one-stop shop" on the road to changing the overall health of our community.
Direct beneficiaries per year: We focused a good deal of our resources helping those adversely affected by COVID.
Geographic areas served: Our service area is 60 miles along the South Coast in Mendocino & Sonoma counties from Irish Beach to Cazadero.
Programs: Using our well trained Community Health Workers, we offer many types of services including Care Transitions (hospital to home), Educational Workshops, Chronic Care Management Support Network, Wellness Screenings, Community Outreach & Referrals, Opioid Prevention, Wellness, & Education.
