Mission: Madonna rehabilitation hospitals is a catholic organization that exists to provide a full continuum of world-class medical and physical rehabilitation services to children and adults throughout the nation. We rehabilitate those who have sustained injuries or disabling conditions such as brain injuries, spinal cord injuries, stroke, neurological diseases and pulmonary conditions so they can fully participate in life; lead research to improve outcomes; and promote wellness through community programs.
Programs: Madonna fulfills this mission by delivering expert and comprehensive rehabilitation, treating each person as a unique individual. Mrh's hospitals integrate the physical, psychological, social, spiritual and vocational aspects of each patient's life. The hospitals' programs support the unmet needs of our community and region and are devoted to returning people to their homes, communities and jobs or school. In fy 2017 madonna served 1,419 hospital inpatients from 18 states. The hospital received referrals from 933 physicians and 95 hospitals or health care facilities. In fy 2017, madonna operated madonna rehabilitation specialty hospital lincoln (mrsh-l), a long term care hospital (ltch) and madonna rehabilitation hospital lincoln (mrh-l), an inpatient rehabilitation facility (irf), madonna rehabilitation hospital omaha (mrh-o), also an inpatient rehabilitation facility (irf). Mrsh-l, mrh-l and mrh-o offer comprehensive care to ensure continuity as patients' progress in their recovery or from one hospital to another. All hospitals employ a transdisciplinary team approach involving the patient and family, referring physician, rehabilitation physician, case manager, nutrition therapist, respiratory therapist, rehabilitation nurse, physical and occupational therapists, orthotic and prosthetic technician, psychologist, social workers, speech-language pathologists, therapeutic recreation specialists, vocational rehabilitation consultants and spiritual care counselors. Madonna's experienced staff treats thousands of individuals through inpatient and outpatient services every year, creating the value that each clinician can offer every patient the latest techniques and methods to produce the best results for every patient. Madonna provides:" intensive therapy programming" programs with less intensive therapy, depending on the needs of the patient" 24-hour rehabilitation nursing and physician supervision; and" a continuum of services to ensure progress is maintained from inpatient to outpatient programsmadonna is accredited by carf for more rehabilitation programs compared to any other facilities in nebraska. These programs are:" comprehensive integrated inpatient rehabilitation program - (mrh-l, mrsh-l, and the transitional care unit (skilled nursing facility)" brain injury program - inpatient and outpatient, adults and pediatrics" spinal cord system of care - inpatient and outpatient, adults and pediatrics" stroke specialty program - inpatient and outpatient, adults and pediatrics" pediatric specialty program - inpatient and outpatient" person-centered long-term care communitymadonna rehabilitation hospital - lincoln (mrh-l) achieved exceptional clinical outcomes this year while serving among the most medically complex persons in the nation (top 2 percent or 98th percentile compared to the national average for other rehabilitation facilities). Patient severity, as measured by case mix index (cmi), was 1. 56, compared to a national average of 1. 35. 81 percent of patients were neurologically impaired compared to just 56 percent nationally. Despite this complexity, compared to erehab industry benchmarks more patients discharged to community, made greater functional gains during their stay, and discharged at a higher functional level. Mrh-l was also in the top 3% in the nation at preventing returns to acute care 30-days post discharge according to cms's most recent pepper report. Despite the medical complexity of persons served, 69 percent of patients discharged to community settings, exceeding the industry benchmark of 67 percent. The stroke, brain injury, spinal cord injury and pediatrics programs all met discharge to community targets for the year as well as targets for keeping return to acute care rates low. Mrh-l patients have also made greater functional gains and discharged at a higher functional level compared to national scores for the functional independence measure (fim). Mrsh-l fy 2017 discharge to community/irf rate was 47. 6% for the year. The industry benchmark for the same period was 41. 81%. To track patient gains, madonna staff surveyed 122 individuals who had discharged to a community setting within six months to one year after discharge, a majority of whom had brain injury, stroke or spinal cord injury. Of these, all 122 were still living in a community setting. Slightly more than 24 percent reported a re-hospitalization although approximately half of those were for planned procedures. Between 92-100 percent of those contacted reported making improvements or maintaining gains since the time of discharge in each of six functional domains including: self-cares, mobility, communication skills, thinking skills, interpersonal skills and emotional functioning. 97. 5 percent reported seeing their primary care physician within the last six months and 99 percent reported being actively involved in some form of ongoing exercise or wellness program. The percentages of people who reported returning to their life roles including work (64. 1 percent), school (100 percent), driving (70. 9 percent), homemaking (95. 5 percent) and other activities were strong, with some areas reporting return at double the rate of comparative populations that are published in the literature. One of the ways madonna achieves excellent long-term outcomes is by focusing on return to school including preschool through college. Madonna's kit scott therapeutic learning center (tlc) is the only onsite, classroom based hospital program of its kind in the region. The tlc helps student patients maintain and, if necessary re-learn academic work, learn new study skills and coping strategies and work with parents and schools to provide the necessary services and support to make a student's transition back to school as successful as possible. Tlc services are offered at no cost to patients. Because it is educational in nature, medical insurance does not cover the cost of patient sessions in tlc while at the same time education funds are not available because the services are provided in a medical setting. The focus on return to academics facilitated by this program has resulted in return to school levels that are currently at 100 percent. In the past when individuals did not return, it was typically due to graduation having occurred during the hospitalization period.
st. Jane de chantal (st. Jane) is comprised of a 38-bed ventilator and special care unit (vau/sn), a 21-bed transitional care rehabilitation unit (tcu) and a 66-bed nursing home. In addition, there is a nine-bed assisted living unit for young adults with high acuity special care needs. The facility provides specialized services under contract for the state of nebraska and the veterans administration. St. Jane offers a holistic approach to care, emphasizing the mind, body and spirit in a welcoming, caring environment focused on improving or maintaining the health and well being of its residents. Fy 2017 continued to focus on resident-centered care with a remodeling project and the completion of new furniture and televisions for the resident rooms. St. Jane continues to offer aromatherapy and a "quiet zone" along with "uninterrupted sleep" which allows for less stimulation and a more restful night. This has had a major impact on our fall rate and a reduction in resident injuries. Both bathing areas feature a resident bathing system including a television for calming videos and music therapy during baths. The st. Jane team continues to focus on increasing non-pharmacologic approaches to minimize pain and anxiety associated with aging and geriatric medical issues. Components include aromatherapy, hand massage, comfort touch , chair yoga, far infrared heat, drum circles, weighted blankets and life celebrations. Madonna uses these approaches in a culture that values the rights, dignity and empowerment of elders. Staff training continued on the techniques of providing comfort touch to residents in all programs. A holistic intervention station continued to be a part of the nurse aide competency training in august 2017, providing education to more than 150 nurses, nurse aides and medication aides on the use of non-pharmacologic interventions. Highlights" in april of fy 2015, st. Jane earned a three-year carf accreditation for inpatient rehabilitation for tcu and person-centered long term care community for the nursing home. St. Jane remains the only nursing facility in nebraska to have earned this level of accreditation. " st. Jane received a 5 star rating through cms nursing home compare (medicare. Gov). " the st. Jane administrator received training through the nebraska health care association (nhca) and became licensed as a nursing home administrator preceptor. There are only approximately 66 licensed nursing home administrator preceptors in the state of nebraska. " st. Jane began preparation to apply for the national silver award through the american health care association, which requires a dedicated team to complete the planning process. " staff at st. Jane remained involved with great plains quality care collaborative (gpqcc) and is highly respected by other long term care facilities throughout the state of nebraska. Staff members presented "improving your bottom line by engaging your front line staff and developed an interactive learning activity on the topic of "stewardship" for the nebraska culture change coalition in june 2017.

Voted number one in physical therapy in the lincoln journal star lincoln's choice awards for the fourth consecutive year. Therapyplus clinicians did an excellent job of providing high quality patient care to maximize clinical outcomes and patient satisfaction despite tighter visit limitations, medicare therapy caps and tighter authorization processes. " the optimal tool scores used to assess outcomes for patients with orthopedic diagnoses improved by 10% (2. 42 points) compared to fy 2016. " completed through the patient assessment, the functional assessment measure (fam) tool determines clinical outcomes in various functional areas for patients with neurological diagnoses. Fam results improved 3% (0. 26 points) compared to fy 2016. " net promoter ratings for patient satisfaction exceeded target at 92% rating their care and experience excellent. other programs include madonna proactive which is lincoln's first medically based health, fitness and wellness facility and the first to be based on a rehabilitation model. Proactive provides medical, therapeutic and educational fitness and wellness expertise in addition to the latest research and technology. The goal of madonna proactive is to meet people where they are regardless of age or ability and help them to become the very best that they can be. Highlights-madonna proactive partnered with the american heart association and chi health nebraska heart for the betteru makeover challenge. A madonna proactive team selected female participants from the community and supported them through a 12-week wellness program including personal training, nutrition counseling, wellness coaching and weekly education sessions. -the life balance for heart/cardiac rehabilitation program graduated 89 patients, which is a 14% increase from last year. -integrative medicine classes included personal consultation sessions and classes for specific medical conditions. Madonna proactive offered a variety of meditation and stress management classes, such as tai chi, to complement a much improved yoga program.
