Above all, Joseph's House offers a caring human presence for men who would otherwise face a lonely, isolated death.
Programs: 24-Hour Nursing Care: Our nurses provide round-the-clock care. They continuously monitor each
resident s health status and ensure modification and implementation of individual care pans. Nurses also serve as liaisons between medical establishments (physicians, hospitals, emergency rooms, and clinics) and each resident.
Personal Care Aide Services: Since our residents vary in the phase of their illness and suffer from multiple physical, mental, or emotional illnesses in addition to AIDS, our certified personal care aides provide a variety of direct care services. All personal care aides are trained and available to provide emotional
support and companionship.
Case Management: Our in-house case manager works with the executive director and nurses to address
client identification, screening and intake. She prepares an individual care plan for each resident, addresses areas of substance abuse, entitlements, supportive services and either transitional support or hospice care.
The case manager assists residents in reestablishing relationships with family members and friends that
have been damaged or severed in the past.
Advocacy, Accompaniment, and Education: Our advocacy includes working with physicians, nurses, case
managers, and administrators in hospitals, hospice programs, prisons, and other service agencies. Through
speaking engagements, newsletters, our website, published writings, and other materials, we share
information about AIDS, poverty, and social justice. The Accompaniment component will extend Joseph s
House s reach to those who are dying, forsaken and alone, by providing formal skills training sessions to
an expanded corps of volunteers who will accompany residents of Joseph s House as well as men who are
dying alone in hospitals, nursing homes, and prisons.
