Programs: Ascentia pathways to community living provided services to approximately 1580 individuals with chronic mental disabilities including mental retardation, severe mental illness, and traumatic brain injury. Services were provided to allow these individuals to function in a community setting while managing their illness
services provided primarily to approximately 475 older adults in their home. Services include personal care, physical therapy,nursing,and occupational therapy. These services allow clients to continue living independently, reducing the potential cost of institutional care.
