I would give this company a million negative stars if I could!! I just read the other two reviews and the similarities are remarkable! I have a few to add however. They denied my mother's death benefits claim after CHANGING THE ELIGIBILITY STATUS FOR THAT MONTH!!!
Not only was that month changed but 4 others as well over a 17 month period. They did the same things to us as they did the gentleman that gave a review before us. Claiming to not have received our paperwork for a total of 12 times in one year! My mother was going through chemotherapy, radiation and a clinical trial and was actually told on SEVERAL occasions that because her unpaid claims amount that was currently due was so high she had to pay up freaking front for all of her scans and treatments! We submitted an insanely high amount of prescriptions for reimbursement as they made us pay up front for EVERYTHING, to this date 6 years later they have not paid a single penny on any prescription whatsoever! We are in debt from this to the point of almost losing our house and all we have worked for our entire lives. The disappointment and disgust I feel towards this company is only growing daily. I called last week and they made me pay almost $1000.00 (they initially had told me $1957.45) and they would release my moms death benefits only to find out yesterday that they applied the money to next month and banked the rest and are not paying after all. We also received a notice in the mail stating we will no longer be eligible for their services as of the first of June.
For the people is a bunch of bull. This place should be ashamed of themselves and each one of them forced to work as janitors for a minimum of 10 years and have all of their benefits completely stripped! It is in my personal opinion as well as the opinion of the oncologist staff at Ohio university James center that they are indirectly responsible for my mother's cancer progression that ultimately killed her. To me that is a crime that needs to be punished. Unfortunately because we've had to pay for so much of her medical the last 6 years we can not afford an attorney, or a loaf of bread. Please Anyone and everyone call whomever you can to force this company to answer for their actions!!!

My husband pays 900 a month an we have to pay for everything upfront an they barely cover ..it!!!!! I am so disgusted with them I have been wearing the same glasses for 10 years. I think I need some NEW ONES!!!!!!!!!!!!!!!!!!!
Last year I was notified by my doctors office that my bills were not getting paid by my Insurance, so I called Indiana state council of roofers health and welfare fund and was told they did not receive my paperwork. This was about 3 months after we sent it in to the address provided and we had not been notified by the Insurance company. So we asked them to mail another copy and we filled it out and we resent it, and once again the doctors office called and said they were still not getting paid, So we asked them to mail another copy. When we received it we filled it out for the third time and sent it to the address provided P.O. Box 5767 Lafayette In. 47903-5769 So once again I contacted the Insurance company and was told they had not received anything from us. Once again I was informed that the bills were not getting paid and were going to collections, so I contacted the Insurance company and spoke with someone in costumer service and ask for the form to be e-mailed to me and she did but said she would need to have the original forms mailed to her and we did this. So I call this morning and I am told that they don't e-mail forms and that their mail goes through a mail sorting company. I told her that I felt that this was a way for them to deny claims because if the mail sorting company does not deliver your mail to them then your claim is denied even though you have done everything expected. I think this company needs to be Investigated for Insurance fraud because I don"t think I'm the only one this happening to.
