Mission: To empower people with disabilities to be their own advocates through peer counseling, independent living skills instruction, advocacy and information and referral, to maximize each individuals capacity for independence in areas including employment, housing, transportation, education and community participation.
Programs: Core provide counseling, independent living skills and advocacy to 344 consumers with disabilities to promote independence, productivity and quality of life in the areas of employment, housing, transportation, transition from nursing facilities to community living, education, among other goal areas. Provide information and referral to approximately 1500 people with disabilities, their families, other service providers and the general public.
hopwa and part b assisted 64 individuals and family members with disabilities to achieve their independent living goals by offering financial rental assistance, housing subsidies and purchase of goods and services that support community based living.
money follows the person aging and disability resource centers assisted people with disabilities and elders to prevent unnecessary placement in nursing facilities, including transition of 59 people living in nursing homes to community based living. Also provided services to 24 elder veterans to obtain benefits and maintain community living.
