Mission: The hudson valley care coalition (hvcc) seeks to improve consumer access, engagement and health outcomes through an innovative, integrated model of care linking social determinants of health with medical and behavioral health services. Hvcc is a collaborative among providers across new york's hudson valley.
Programs: Hvcc works with the department of health, managed care organizations, community providers and medical centers to improve the health outcomes of medicaid patients diagnosed with chronic health and behavioral health conditions.
