Mission: The hospice family care foundation, inc. Is a charitable, grant-making organization dedicated to improving the quality of life for the terminally ill, their families and support systems by providing financial assistance, medical and non-medical goods and services and reimbursement to hospice providers when alternate payer sources are not available and the patient meets all eligibility requirements. The foundation is also committed to providing education to the medical and lay communities in the unique benefits of the hospice philosophy, provide scholarships to healthcare workers to continue their professional education and provide bereavement support services when those needs exceed beyond the hospice program's responsibility as defined by medicare and state regulations.
Programs: The foundation provided financial assistance to numerous hospice patients and their families. The total number of patients served in 2011 was 97 patients for burial/cremation services, 44 patients needing financial support for utilities, 33 patients needing food vouchers, 47 patients needing financial support for housing, 38 patients with other family requests, 46 patients needing hospice medical care. In addition, 376 patients were served thanksgiving and christmas dinners.
