Mission: The mission of hospice at home is to serve our patients, families and communities with dedication and compassion; to deliver the best end of life care to help people experience peaceful, pain-free and sacred deaths within the context of their own lives; and to guide our youth, adults and families through the grief process with timely and consistent availability.
Programs: Comprehensive services - services largely purchased by medicare/medicaid providing skilled nursing, social work, home health aid, therapy medication, dressings, lab, medical, equipment, etc for the terminally ill.
bereavement services - nursing, social work and bereavement counseling, support groups and supportive correspondence for the bereaved family members of hospice patients.
supportive services - clinical support including clinical administrative assistants, leadership, and staff with direct clinical oversight.
other program services include education services and volunteer services.
