Causes: Mental Health, Substance Abuse Treatment
Mission: Horizon house rehabilitation services, inc. Is one of the three service-delivery affiliates of horizon house, inc. The continuum of community-based services delivered by this horizon house affiliate includes mental health and drug and alcohol residential, homeless, behavioral health treatment (mental health and drug and alcohol outpatient, drug and alcohol intensive outpatient, partial and medication management) and rehabilitation (employment, education and case management). All services provided by horizon house rehabilitation services, inc. Are designed to assist individuals with psychiatric disabilities, histories of substance abuse or homelessness to learn skills to perform daily living tasks, build social and vocational competencies, manage their resources, and reach their highest potential for independent living in the community. These services are provided in philadelphia, chester and delaware counties, pennsylvania.
Programs: Residential services - transitional and permanent, ranging from highly structured long-term group homes to staff supported independent living options.
