Mission: Horizon health and wellness inc's mission is to provide strength-based, culturally sensitive integrated health services by partnering with families and individuals to manage challenges and support their recovery. Hhw developed an (cont on schedule o) innovative proactive model that embodies our philosophy of whole person care while promoting wellness. This approach to health care addresses the whole person by integrating behavioral health with primary care while promoting wellness and prevention.
Programs: Providing comprehensive integrated health services for adults, childrenand families including assessment, evaluation, counseling, psychiatricand primary care services. Our services also include intensiveoutpatient treatment, crisis intervention inpatient evaluation andtreatment, outreach and domestic violence services. We provideprograms to increase community involvement, improve socializationskills and encourage independence. Residential services and assistedliving programs are also provided. Our services and programs are madeavailable to more than 10,000 consumers annually in pinal, gila, maricopa, cochise, santa cruz and yuma counties.
